This is an example of how to fill out the form hat was issued to you from our Dr Lock locksmith after you have had a Security key system installed.

The 4 main points are.

1. List an address that the keys will be posted to. This can not be the same as the address of the property that the security keys work. Most people use a P.O box or office address.

2. You will need to place your signature in the box. This is so we can varify your requset for more keys in the future. Note if your request do not have the authorised signature we will not cut the keys.

3. Your details of the authorised person or persons. Please provide a business card if posiable, and please fill out all the spaces, they must be readable !

4. Simply fax it back or post it to us.
